MODU APPLICATIONS

Applications should contain the following:

1. Seamen applications with medical forms not older than 2 years, and drug test results not older than 1 year.

2. Two (2) color photographs.

3. For officers, copies of their US coast guard licenses, or written assessments, with required course certificates as required for the rank they are applying for.

4. Ratings (AB, MOTORMAN, & OILER) must have course certificates as per STCW requirements for the respective ranks.

    Note: All seamen applying for lifeboat men must submit letters of service with the rank they hold on the vessel.

5. Reference Letter from Owner or Manning Agency.

6. A cover letter containing the vessel name and names of the seamen who are applying for Vanuatu documents.

Once an application is complete, the authorized person(s) must sign and date the form in the appropriate boxes. Although you may submit your application online for expedited service, THE ORIGINAL SIGNED FORM MUST BE MAILED to the Deputy Commissioner office.
