MODU APPLICATIONS

Applications should contain the following information:

1. Seaman application with medical not older than 2 years, drug test results not older than 1 year

2. Two (2) color photographs

3. For officers, copies of their US coast guard license, or written assessments, with required course certificates as required for the rank they are applying for (see MODU requirements)

4. Ratings (AB, MOTORMAN, & OILER) must have course certificates per STCW requirements for that rank.

Note: all seamen applying for lifeboat man must submit a letter of service with the rank they held on to the vessel.

5. Reference Letter from Owner or Manning Agency.

6. Cover Letter containing the vessel name and names of the seamen who are applying for Vanuatu Documents.

Once the document is complete, authorized person(s) must sign and date the form in the appropriate boxes. Although you may submit your application online for expedited service, THE ORIGINAL SIGNED FORM MUST BE MAILED to the Deputy Commissioner office.
